Hmm this is tough...the girl looks like she's in more pain than the guy.
I'd guess the second one would be the medical one, but that depends if you've got a cunning twist in store for us or not :p

Not helping - how uncomfortable the first model looks. She LOOKS like she needs the bottom item.
For $170 dollars...that thing best be a neck cowl, a brace, AND help pay my rent.
